Online Booking for Established and Transfer Patients
For established and transfer patients, requesting a telehealth appointment via online booking is available via the Patient Fusion portal. You will be invited by our team to activate your portal account following completion of LiNC registration (see Interested in Care Tab above).
Once you have activated your portal, you can login to your portal account, click on "Providers" and select "Book an appointment." You will receive confirmation of the request when approved or an alternative time suggestion if that time is no longer available. We may not be able to confirm appointments made less than 48 hours in advance.

Reaching your Physician if you are an established patient
Please contact your LiNC physician via our electronic medical record portal (Patient Fusion) for the following concerns or requests:
Schedule a Follow-up Visit
Request a Medication Refill
Request a Form or Letter
Send a message to your doctor
Other communication need
We generally check messages once daily, and you can anticipate response within 2 business days or sooner (weekends and LiNC holidays excluded). If you need assistance with Patient Fusion, please contact our office at info@linccenter.com.
Please note, we are not an urgent care facility and we do not provide emergency support services for crisis situations.
For urgent or crisis situation, please call 911 (Emergency) or 988 (Suicide and Crisis Lifeline) or seek care in your local hospital’s ER.
