How to establish care
Registration Forms:
To register as a transfer patient:
Transfer patients are individuals seen previously by Drs. Marco, Steele, or Shapiro in another clinical setting within the last 3 years.
For transfer patients ready to schedule intake visits, please complete the following forms (above):
- LiNC Registration Form
- Billing Information Form
- Authorization for Release or Exchange of Medical Information (for Transfer Records)
Appointments for transfer patients:
After receiving your registration documents, we will share portal access via email to allow you to self-schedule your transfer follow-up appointment.
To register as a new patient:
For new patients ready to schedule intake visits, please complete the following forms (above):
- LiNC Registration Form
- Intake Form (based on age, we have a pediatric form and an adult form)
- Billing Information Form
- (Optional) Authorization for Release or Exchange of Medical Information
Appointments for new patients:
Once we have received your registration documents, we will be in touch to schedule appointments or confirm that you are on our waitlist.
As of January 2025, current wait time for assessment is approximately 6 - 9 months.
For patients who have a scheduled intake or are on our waitlist, you will be notified if earlier appointments may become available due to cancellations
For most pediatric new patients, our intake process is a two-visit set, visits scheduled one week apart.
For most adult new patients, our intake process is a single visit with follow up care scheduled as needed at the time of intake.

Insurance Reimbursement information:
We do not bill insurances directly and can not guarantee insurance coverage. Patients are responsible for payment of clinic charges following visits. You can contact your insurance provider to inquire about reimbursement.
